Refer a Patient for Memory and Dementia Evaluation  

The Louis and Anne Green Memory and Wellness Center in Boca Raton welcomes referrals from physicians and other healthcare providers for patients experiencing memory loss, cognitive decline, or other changes in memory and thinking.  

As a State of Florida-designated Memory Disorder Clinic, our interdisciplinary team provides comprehensive memory evaluations and specialized services for individuals with concerns related to mild cognitive impairment, Alzheimer’s disease, vascular dementia, Lewy body dementia, frontotemporal dementia, and other related dementias.  

Beyond evaluation and diagnosis, our memory clinic helps patients and their families understand the next steps, navigate care planning, access care partner support, and connect with appropriate services and community resources.
