Sustaining the Care Partner Program
Dementia Caregiver and Care Partner Support
The Sustaining the Care Partner Program at the Memory and Wellness Center offers a range of services designed to support spouses, partners, adult children, other family members, and friends who care for or assist individuals living with memory disorders, Alzheimer’s disease, and related dementias (ADRD) .
Caring for someone with dementia or Alzheimer’s disease can be challenging, frustrating, and sometimes confusing. Education, information, and caregiver support can help both the person living with a memory disorder and their care partner maintain the highest possible levels of health, well-being, independence, and quality of life.
Our dementia caregiver support services are designed to address the individual needs of families and care partners at different stages of the caregiving journey. Many of these programs are offered at no cost through generous grants, gifts, and support from our Caring Hearts Auxiliary.
Care Partner Support and Services
Care Consultations
Care consultations provide individualized caregiver and care partner support based on “what matters most” to the individual and family at the time of the consultation.
A staff member works with care partners to provide information, resources, and practical strategies that can support positive approaches to dementia care and Alzheimer’s care , while also promoting the health and well-being of the caregiver.
These are typically one-time, issue-focused consultations provided on an as-needed basis and may address concerns related to caregiving, changes in behavior, daily functioning, communication, safety, or other challenges associated with memory disorders.
Psychotherapy and Counseling
Psychotherapy and counseling are available to individuals of all ages who are seeking healthier, more effective coping strategies and habits.
Dementia caregiver counseling is also available for care partners experiencing emotional challenges related to changing roles and responsibilities. Support may address feelings of sadness, depression, anxiety, stress, or other emotional concerns associated with caring for someone with memory loss or dementia.
SPA for Care Partners
Self-Preservation Activities (SPA) for Care Partners provides education and support to help caregivers better understand the challenges associated with caring for someone with a memory disorder.
These dementia caregiver education programs provide practical information and resources to help care partners feel more prepared to navigate the challenges of dementia care and include topics such as understanding the diagnosis of dementia, medication issues, handling behavioral and psychological changes, Advance Care Planning (ACP), recommended lifestyle interventions, and managing stress.
