Behavioral health services are provided directly and integrated with primary care. Adolescent and adult patients are screened for depression, at intake and at least annually thereafter. The behavioral health professionals are available to work with patients who need assessment of their behavioral health needs. The goal of these integrated services is to develop and implement, within primary care, brief problem-focused treatment plans that include both medical and behavioral interventions focused on restoration of function and quality of life. Patients learn behavior strategies to help them adhere to medical regimens, new coping mechanisms, stress reduction strategies, and pain and/or disease management techniques. The behavioral health team includes psychiatric mental health nurse practitioners, family nurse practitioners, social workers, holistic health providers and a psychiatrist.
